CRNA Application: Nursing Exam Case Management Information Required

I am sending you this email in preparation for the assessment of your request for an additional exam attempt. Please provide the following documentation by _____________.

1. Letter addressed to the Registration Committee: 
   1. explaining the extenuating circumstances that affected your ability to write the exam 
   2. when determining if there are extenuating circumstances, the CRNA will consider, whether the circumstances were extraordinary, unexpected, may have been addressed by you and are sufficient to justify making an exception for an additional exam writing 
   3. how you will address your circumstances should you be granted an additional writing 
   4. when you will be ready to attempt another examination 
   5. how you would prepare, if granted an additional writing 
2. Provide evidence related to the information you provide in your letter (e.g. medical documentation, confirmation of course attendance, plan to prepare for an additional writing, etc.). 
3. Complete the attached timeline template as it is designed to support you record specific details surrounding the examination attempts. 
4. Any other pertinent information to support your request.

You can scan documents (word or PDF preferred) and email them to casemanagement@nurses.ab.ca or fax them to 780.443.6582.

Please note failure to meet the above deadline may result in a delayed decision and/or a decision based on the information we have on file currently.

Our decision could be to approve or refuse your request for an additional exam attempt.
