CRNA: Registrar Prep IEN NP

Hello {{RegistrantFirstName}},

As a follow-up to our conversation, I am sending you this email in preparation for the upcoming Registration Committee meeting. Please provide the following documentation by _____________ for the _____________ Registration Committee meeting:

1. Letter addressed to the Registration Committee:
   1. outlining your nurse practitioner (NP) education
   2. describing your work experiences as an NP
   3. proposed planned preparation to ensure competence for NP practice here in Alberta
2. Completed attached self-assessment of NP competencies form.
3. Course descriptions of the courses that you completed during your NP program.
4. Arrange for your most recent employer(s) to complete the attached employer reference and practice references on your NP practice and submit directly to the CRNA (please note the NP practice reference must be completed by an NP or MD who can attest to your competencies).
5. Copies of your NP exam results and/or current certifications with NP certification boards.
6. Copies of your official NP education transcripts.
7. Any other pertinent information to support your request.

You can scan documents (word or PDF preferred) and email them to casemanagement@nurses.ab.ca or fax them to 780.443.6582.

Please note that failure to meet the above deadline may result in the deferral of your case to a future Registration Committee meeting and/or a decision based on the information we have on file currently.

The decision of the Registration Committee could be to approve, approve with conditions, or refuse your application for a NP permit.
