Following the review of the Registration Committee, your request for one or more attempts to pass the RN registration exam has been approved. You are eligible to attempt the NCLEX-RN exam one or more times to be written on or before _____________. 

If you have not already, please apply to Pearson VUE to register for the NCLEX-RN, obtain you authorization to test (ATT) and schedule your exam appointment. In the event you are not successful on your next attempt at the exam, or your ATT expires, you may apply to CARNA and/or Pearson VUE again and schedule another attempt to be written on or before _____________.
