CRNA: Registration Committee Preparation

Hello {{RegistrantFirstName}},

As a follow-up to our conversation, I am sending you this email in preparation for the upcoming Registration Committee meeting. Please provide the following documentation by _________________ for the _________________ Registration Committee meeting:

1. Letter addressed to the Registration Committee:  
   1. explaining the circumstances that resulted in the criminal charges and current status in regard to court-imposed sanctions  
   2. court documents pertaining to the charges, proceedings and court-imposed sanctions  
   3. evidence of your compliance with court-imposed sanctions  
   4. what you have learned from the situation as well as any additional activities you have independently undertaken to address the circumstances that led to the criminal conviction, etc. 
2. Provide evidence related to the information you provide in your letter (e.g. completion of required remediation, fine payment, court documents, past criminal records checks, etc.)  
3. Provide a Criminal Records Check completed within the last three months. 
4. Arrange for your most recent employer(s) to complete the attached employer reference on your RN practice and submit directly to the CRNA.  
5. Any other pertinent information to support your request.

You can scan documents (word or PDF preferred) and email them to casemanagement@nurses.ab.ca or fax them to 780.443.6582.

Please note that failure to meet the above deadline may result in the deferral of your case to a future Registration Committee meeting and/or a decision based on the information we have on file currently.

The decision of the Registration Committee could be to approve, approve with conditions, or refuse your application for a RN permit.

You may continue to work on your _____ RN practice permit until the Registration Committee makes a decision on your _____ RN practice permit application.
