CRNA: Security Enforcement Permit Application Locked

Hello {{RegistrantFirstName}}, 

Your application with the CRNA has been locked due to suspicious activity or suspected fraud. It appears that you are either using a third-party agency or having another individual complete your application for you, which is not permitted.

To unlock your account, you must complete the following steps:

Call 1.800.252.9392 to speak to a customer service expert to book an appointment with a registrant navigator. You cannot book this appointment through email or the form on the CRNA's website. 

The meeting you book with a registrant navigator will be a video call via MS Teams. In this meeting, you will be required to show proof of your identity using valid, government-issued identification. 

You will be unable to proceed with your application or receive a practice permit from the CRNA until you have met with a registrant navigator. 

All applicants are required to complete the application process and attest to meeting requirements themselves. Individuals who have been found to provide false or misleading information as part of their application will be denied a permit.
