CRNA: Your Name Change Virtual Meeting Invitation

You are receiving this email because your name change through our third party, Trulioo, has had two unsuccessful attempts. To continue your application, you will need to meet virtually with one of our Customer Service Experts to complete the identity validation manually.

Virtual Identity Validation Meeting Booking 

This meeting will be hosted on Microsoft Teams, you must attend yourself (no representatives), and you must have your camera on during the call to confirm your identity. Please ensure you have your government-issued ID ready, as you will be asked to show it to the Customer Service Expert during the meeting. For a list of approved government-issued IDs, please click here. 

This meeting must be completed before we are able to continue with changing your name on file with the CRNA.
